In this seminar we will discuss the basics of cross-professional care for chronically ill people. Chronic care includes a transdisciplinary approach. Theories and concepts from different disciplines, such as social work, social sciences and nursing science, are discussed. Looking at care processes occurs in the seminar from the perspective of the person concerned. In the context of in-depth group work phases, we will also discuss aspects of the treating organizations, transitions between organizations and the overall social framework. Topics of the seminar will include: subjective experience of chronic illness, social determinants of illness, trajectories, approaches to person-centered care, resilience, coping and empowerment.
